1. Applicant must come personally with their passport and complete Form A (IM42) available at £ 0.25 pence each.

2. Original Malaysian Identity Card (I.C)+ One Photocopy of each side of the I.C and Birth Certificate or Borang W if born outside Malaysia

3. 2 (Two) Passport Photos (Blue Colour Background, Full Face facing the camera and decent dress (No bare chest). (3.5 cm x 5.0 cm)

4. If the Father or Mother is not avaialble for the reason like Father and Mother residing in different country then the Guardian need to come with A1/B1 form that has been signed and stamped by Immigration Officer or Authorised Officer at any Malaysan Mission Overseas and they have to bring their original I.C and a photocopy

5. Cash Payment of £ 50.00 per 32 pages passport and £ 100.00 per 64 pages passport.

6. All applicant must come to the Immigration Office between 09.00 am to 12.00 pm to get the queue number.
