Report of Missing Documents

Report the loss at the nearest Police Station

Documents such as Birth Certificate, Malaysian Identity Card / MyKAD and Driving License of Malaysia that have gone missing or were lost need to be reported to the local Police Station and the High Commision of Malaysia:

     -    A copy of the police report
     -    A copy of the Malaysian Passport
     -    Other documents that can be used as proof of citizenship
     -    A self-addres envelope with stamps
